Ot SERVICES
OT EVALUATION
Comprehensive caregiver intake, in-person assessment, and a meeting to review the evaluation report & therapy goals. We recommend this option if your child has never received OT before, or if it has been more than 1 year since their last evaluation.
$350 flat rate
OT SCREENING
Simplified caregiver intake and assessment. Does not include written report, but OTR will review interpretations with caregiver. We recommend this option if your child needs an updated or additional assessment to receive services such as our OT Groups.
$175 flat rate

THERAPY SESSIONS
Personalized and collaborative occupational therapy sessions that are child-led and family-centered, occurring in the natural environment at home or out in the community.
CONSULT SERVICES
Consults may occur in-person or virtually, either before/after or outside of therapy sessions. Involves ongoing education, guidance/support, and other services requiring OT expertise.
Therapy & Consult Services are $100/hour if choosing to pay-per-service. See below for our package options!
Our package options ensure your child & family receive best combination of all the essentials to create lasting change. Packages provide flexibility to customize services, as well as progressive consistency to carryover skills across natural environments.
NEW MOON PACKAGE
This package works best for children who receive either weekly or biweekly OT services and families looking for as-needed support/education to carryover between therapy sessions.
4 hours per month that can be used for both therapy sessions and consult services. You get to decide the combination of services that best fits your family's needs each month. Hours can be rolled over to the following month if there's a cancellation or if all hours do not get used during the month.
Example of 4 hours monthly usage:
Four 45 min sessions + 1 hr consult (45 min + 15 min weekly)
Two 1 hr sessions + 2 hr consult (60 min biweekly + 30 min weekly)
Monthly Rate = $380 total ($95/hr)
FULL MOON PACKAGE
This is our most recommended package because it offers the ideal combination of consistent sessions, ongoing education and care coordination, and interactive family guidance/support. This package works best for children who receive weekly OT services and families looking for ongoing support to carryover in between therapy sessions.
5 hours per month that can be used for both therapy sessions and consult services. You get to decide the combination of services that best fits your family's needs each month. Hours can be rolled over to the following month if there's a cancellation or if all hours do not get used during the month.
Examples of 5 hours monthly usage:
Four 1 hr sessions + 1 hr consult (60 min + 15 min weekly)
Four 45 min sessions + 2 hr consult (45 min + 30 min weekly)
Monthly Rate = $450 total ($90/hr)
A travel fee will be applied to the overall hourly rate if travel is more than 30 minutes or if transportation is provided by the clinician.
