FAPT / CSA clients

If you are a case manager seeking an occupational therapy evaluation for a child, please use the links below to submit a New Referral Form and book an Initial Case Consult Call with Vaz at the link below. Please follow these two steps *BEFORE* an official request is made to FAPT/CSA, so that it allows us time to determine if the referral is appropriate.

Additionally, the caregiver will need to complete and sign our OT Registration Forms found under FORMS. It is MANDATORY that these forms are submitted before the initial appointment.
