Out of Network 

Full Moon Pediatric Therapy is primarily a self-pay practice, with the exception of FAPT/CSA-funded services. This allows our independent practitioners to devote sufficient time and resources to provide occupational therapy services in your child's natural environments, while also maintaining modest caseloads. Self-pay also allows for increased flexibility and collaboration, as well as an unlimited amount of sessions to best address the specific needs of your child/family without the barriers that may come with insurance. 

We do NOT accept insurance as a form of payment, including Medicaid. However, an itemized superbill containing all necessary information and details can be provided for you to submit to your insurance company, as well as any supportive clinical documentation if needed. Since insurance companies vary widely in their deductibles and reimbursement for out-of-network providers, it is highly recommended that you talk at length with your insurance company prior to making a commitment with us, so that you have an idea of what your final out-of-pocket expense will be. There is NO guarantee that you will receive any reimbursement. It is the legal guardian's responsibility to submit the itemized superbills provided and to seek insurance reimbursement. 

*Please note that consult services and skills groups cannot be included in a superbill as they are not eligible for out-of-network reimbursement.*

Payment Methods

Payment is due at the time a treatment or group package is purchased, or when individual services are rendered if choosing to pay per service. Full Moon Pediatric Therapy uses a high-security, convenient payment processing platform called Square, which allows for both point-of-service and online payments. Credit/debit cards, HSA/FSA cards, and ACH bank transfers are accepted through Square.

Good Faith Estimate

Full Moon Pediatric Therapy will provide you with a "Good Faith Estimate" explaining the expected costs for occupational therapy services. You have the right to receive a “Good Faith Estimate” explaining how much your therapy services will cost. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the expected charges for medical services. You can ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.
